your medical care at the
end-of life? There may
be a time in your life
when you are unable to
make health care decisions
for yourself. Ask
yourself the following
questions: Are there
types of medical care
you wish to receive or
not receive? Is there a
person in your life you
trust to make health
care decisions for you?
There are steps you can
take now to guide your
health care team and
loved ones during times
you cannot make decisions
for yourself. This
is called Advance Care
Planning. Health Care
Proxy and Living Will
are two types of Advance
Care Planning.
Health Care Proxy
A health care proxy
allows you to choose a
person to make health
care decisions on your
behalf if you are not able
to. This person must be
over the age of 18 and
can be a family member,
close friend or anyone
else in your life. A
health care proxy form
is available at the New
York State Department
of Health website. Before
appointing a proxy,
discuss the medical care
you wish to receive and
confi rm they are willing
to make health care decisions
for you. Fill out
the form with instructions
for your health
care proxy to follow. You,
your proxy and two witnesses
must all sign the
form. Provide a signed
copy of the form to your
proxy, primary medical
provider, and keep one
with you.
Living Will
A living will is a legal
document that outlines
medical treatments you
want to receive if you are
unable to make healthcare
decisions. A living
will indicates your preferences
for treatments
such as dialysis, breathing
machines, tube feeding,
resuscitation, and
organ donations, among
others.

Care Planning
Documents
To learn more about
Advance Care Planning
contact your primary
care provider, the health
department or call 311.
Once you have completed
your Advance Care Planning,
be sure to review
your decisions from time
to time. You may want to
adjust your preferences
for care if your situation
or health changes. Be
sure to inform your doctor,
proxy and loved ones
of any changes.

As you might have
heard on TV or read in
our local newspaper,
the mayor has approved
the request for a ferry
in our area.
The ferry will be
launched from Ferry
Point Park. Specifi cs
were not made available
to me and that is why I
have asked Councilman
Mark Gjonaj, who was
instrumental in making
it happen, to come
to our next meeting. He
has accepted the invitation.
The date is April
30 at 7:30 p.m., at the
Villa Barone Manor,
located at 737 Throgs
Neck Expressway.
I am encouraging
Ferry Point residents
as well as Throggs Neck
residents to attend this
next meeting. Feel free
to ask questions, express
possible concerns
and hopefully get answers.
Our meeting is
held together with the
Throggs Neck Homeowners
Association and
once again we are very
grateful to them for allowing
us this opportunity.
