Assisted Living Program On The Od
A NEW MEDICAID ENRICHED Housing
Assisted Living Building is under construction on
the Rebekah campus scheduled to be completed
July, 2019. Project costs of $16,250,000 were off set
by a $6,500.000 grant from the NYS Department
of Health.
The ALP will provide an essential link in the continuum
of care functioning as a desirable alternative
to the skilled nursing facility. For many frail
elderly whose health care needs exceed the capacity
of home care services and who are eligible for nursing
home care, this supportive residential model
will best suit their housing and health care needs
in an environment that fosters independence and
self esteem. The ALP will consist of 56 attractive
units with kitchenettes and handicap accessible
bathrooms with common areas, such as lounge/dining
area conveniently located. Residential services
provided will include room board, meals, housekeeping,
supervision, personal care and activities.
Residents will be off ered a coordinated program of
case management, recreational activities and health
care services in an environment that maintains stability
and encourages self direction.
The unique feature of this ALP will be the ability
of residents to access the diversifi ed array of community
based services and activities off ered by Rebekah
Rehab. Just a few steps away at the RHCF,
residents will be able to participate in the senior
center programs, intergenerational programs, recreational
and wellness programs, and much more.
ALP residents will be encouraged to participate in
their individual plan of care which will be aimed
at promoting the social, physical and mental wellbeing
of the residents. All residents will receive case
management services that will assure that the plan
of care is tailored to individual needs and that the
resident is receiving the appropriate level of care.
The case manager will be available to assess resident
need, assist with accessing outside services
