such as primary and/or acute care, rehabilitation
therapy and ensuring the residents’ maintains their
independence. Through the coordination with Rebekah
Rehab Licensed Home Care Services Agency,
the case manager will have access to needed medical
and rehabilitation services in a timely manner
to avoid unnecessary hospital stays and precipitous
nursing home placement. The case manager will assist
in scheduling appointments and ensuring that
transportation is available.
Residents will come to the ALP with an established
physician and primary care network will be encouraged
to continue that relationship. If a resident is in
need of a primary care physician or ancillary care
such as podiatry or laboratory services, the case
manager will assist in securing the services of an appropriate
health care professional. ALP residents
who do not have their own physician will have access
to primary care services through an existing relationship
with attending physicians currently on staff at
Rebekah Rehab, as well as through specialty clinics
operated at the nearby health facilities. Dental
services will also be available through these sources
which are nearby, and case managers will assist in
scheduling appointments and arranging transportation
on an as needed basis. Should the need arise
for nursing services, arrangements will be made with
the Rebekah Rehab Certifi ed Home Care Program
(CHHA) for the provision of such services. Should
transfer to a nursing facility become necessary, the
case manager will arrange for nursing facility placement
either at Rebekah Rehab or other community
residential health care facilities as appropriate.
Personal care staff will be available 24 hours per day,
seven days a week to assist with dressing, bathing, toileting
and transfer as necessary. The personal care workers
will also assist with self administration- of medications if
needed and will help residents in the dining room.
