Guest speakers include Valentine Hernandez, Executive Director, QCCP (Queens Coordinated Care Partners), a health home, Lisa Cole, NAI Coordinator, and Wai Ping Kan, outreach specialist from New Americans Welcome Center, an agency developed by YMCA to help immigrants �������������������������������������������������������������������������������������������������������������������������� The speakers will discuss the following: • The health home program’s role in providing an individual with needed medical, social, and educational services. • Health literacy workshops that provide access to doctors, health insurance, emergency Medicaid, and referral information for additional resources. The NYP Queens PPS application was approved in March 2015. This is one of 25 such plans accepted by the Department of Health for implementation in New York State. The NYP Queens plan is working to address some of the most urgent health care needs of our community through integrated, high-performing health care delivery projects.
