Center for Advanced Endoscopy, SUNY Downstate Advanced Endoscopy in the diagnosis and treatment of Gastrointestinal problems Shivakumar Vignesh, MD Chief of the GI Division and Director, Center for Advanced Endoscopy at SUNY Downstate At the Center for Advanced Endoscopy at SUNY Downstate we have several advanced endoscopic tools that improve our ability to diagnose and treat Gastrointestinal conditions. We offer these services both at the University Hospital of Brooklyn and at the Ambulatory Center at Bay ridge. Acid reflux affects about 60% of the US population and increases risk for esophageal (food pipe) cancer. Upper endoscopy (flexible tube with a camera) is helpful in detecting acid induced damage to the esophageal lining and precancerous changes in the food pipe called Barrett’s esophagus. We can diagnose precancerous changes without doing a biopsy, by using a special microscope in the tip of the endoscope called Cellvizio™. This helps us immediately diagnose the precancerous change and simultaneously offer endoscopic treatment. At Downstate we also possess the Barrx device to endoscopically treat precancerous changes of the food pipe using radiofrequency (heat) energy. This is a simple outpatient procedure that is well tolerated. Diseases of the bile duct and pancreas are a common cause of abdominal pain and may result from inflammation, scar tissue or cancer of these organs. At Downstate, we have state of the art ERCP equipment that includes Spyglass™ (Baby endoscope for the bile duct). With this technology we can insert a tiny endoscope into the bile duct that enables direct visualization of abnormalities and improves diagnosis of cancer of the bile duct by direct biopsy. The Spyglass technique also enables endoscopic treatment of bile duct cancer by Radiofrequency ablation (heat) or Photodynamic therapy (a special laser). The Cellvizio™ (endoscopic microscope) is also used with the Spyglass for the bile duct. The same technology is also helpful for early diagnosis of pancreatic cancer and precancerous growths in the pancreas. Endoscopic removal of early stage cancers using a technique called Endoscopic submucosal dissection (ESD) is available at SUNY Downstate. Dr.Vignesh received special training for this at the Tokyo national cancer hospital. Special endoscopic knives are used for safely removing cancers of the food pipe, stomach and colon after initial endoscopy determines that the cancer is at an early stage. This avoids major surgery and it’s risks. Endoscopic removal of cancer is usually a same day procedure but patients may be monitored overnight depending on the situation.
